The Complex Medical Help Program (CMH) is a health care program in the Ohio Department of Health (ODH). CMH links individuals with special health care needs to a network of quality providers and helps individuals and their families obtain payment for needed services.

CMH's mission is to assure, through the development and support of high quality, coordinated systems, that children with special health care needs and their families obtain comprehensive care and services that are family centered, community based and culturally sensitive.

FCHD public health nurses assist families with the application process, health education, case management and referrals to local resources.

OHIO DEPARTMENT OF HEALTH CHILDREN WITH MEDICAL HANDICAPS (CMH)

The Complex Medical Help Program (CMH) provides assistance to children with special health care needs and their families who meet the medical and financial eligibility criteria. Assistance is provided to children under the age of 23 who have special health care needs and are residents of the State of Ohio.

PREVENT BLINDNESS OHIO

The Ohio Affiliate of Prevent Blindness is celebrating 65+ years as Ohio’s leading volunteer nonprofit public health organization dedicated to preventing blindness and preserving sight. PBO serves all 88 Ohio counties. We provide direct services to more than 1 million Ohioans annually and educate millions of consumers about what they can do to protect and preserve their precious gift of sight.

SAVE OUR SIGHT PROGRAM / OHIO AMBLYOPE REGISTRY

The Save Our Sight Fund is administered by the Ohio Department of Health . The purpose of this program is to keep kids eyes healthy with prevention, screening, education and support.

The Ohio Amblyope Registry is part of the Save Our Sight program that receives voluntary contributions from the check-off donation box on Ohio license plate renewal forms. The Registry is a partnership among Ohio’s eye care professionals and the leading children’s hospitals across the state of Ohio.
